This plan is designed for individuals whose lifestyles center around good health and disease/pain prevention.

Package includes:

• Yearly Wellness Exam (includes physical exam) – $250
• 6-month Checkup Visit – $150-250
• 12 x monthly B-12 injections (Methylcobalamin, the Bio-Activated form of B12) at clinic or mobile – $360 ($30 each) x 20% = $288
• 12 x monthly chiropractic treatments – $720 ($60 each)
• 10% discount on any other office visits during the year
